Before beginning treatment with us at Kip, it is important that you understand our practice policies, including your rights to privacy (e.g., HIPAA), and acknowledge that you’ve received these documents with a signature. Please read and sign the documents below, and download them for your own records.

Our mission at Kip is to provide the best possible treatment to our clients, as well as provide rigorous training and supervision to our therapists. As part of this mission, we ask clients to volunteer to have their therapy sessions recorded. We find that these recordings are invaluable tools in understanding what makes therapy work and how we can improve our services. We do not record sessions without our clients’ consent, which you can indicate by reviewing and signing our consent form.

All clients are required to provide credit/debit card information which will be used to charge for sessions at the time that services are rendered.
